New Patient Registration
Welcome. Before your first appointment, please register yourself or your child/adolescent with us.
This process should take approximately 10 minutes, and will provide the basic information about you to our clinicians before your initial meeting. While most questions are optional, if you see * next to the question, it is a required information. You will need to enter the information before you can move forward in the process.
You will be asked to create a username/ID using your mobile phone.
IMPORTANT - you will be asked to e-sign using DocuSign at the end of the process. Please select the digital signature and click through all of the signature areas until you see the DOWNLOAD button. It is not complete until you get to this point.

Registering with PDF/Paper Forms
These forms will give you an opportunity to provide some basic information about you to our clinicians before the initial meeting.
OPTION 1: Fill in the information directly on the Adult online pdf form, or Child/Adolescent online pdf then print.
OPTION 2: Download the pdf , then fill in your responses with a pen.
Bring the printout with you to your initial meeting.

To access your upcoming schedule, secured communication from your physician, and important paperwork and documents, click to the patient portal. There are a few legal forms which we need to have in place to comply with regulations and to to protect you as our patient. Feel free to browse through these and submit one or more if requested by your clinician
Billing and Payment
To make payments, provide your account at Mind Therapy Clinic. If you do not have your account name, enter your name. After clicking on Pay Now button, enter the amount you’d like to pay.
